I'm now coming to the conclusion that Joe was kept in the dark about his physical condition. His own people gaslighted (gaslit?) him. The people around him told him he was fine. Did they do PSA tests? If they drew blood he wouldn't know what tests were done. If they showed him lab result were they the actual lab values? He saw the talking head statements about how vigorous he was. So he thought he was doing great - ready for a second term.

He was led down the garden path by the people he trusted the most - particularly Dr. Jill.

Yes, but only because all of them, to a man, are scum-sucking losers who only care about themselves and the power they wield over people's lives.
