It is possible the two year pulled on the pistol and the trigger snagged on something activating it.

I have tested with mine. Just hold hold it in the left hand and push two outstretched fingers of the right hand through the trigger guard at a right angle and it fires.

My thought is that it was probably a small-caliber semi; she saw him with the gun after he pulled it out, and was going towards him when he fired.
